Why Am I Seeing This Information?
As part of new Medicare requirements starting January 1, 2027, Medicare Advantage health plans will participate in health information sharing programs designed to improve care coordination, continuity of care, and other healthcare activities.
These programs give you choices about how certain health information may be shared with healthcare providers involved in your care and between health plans.
Each program works differently and gives you different choices. The sections below explain how each program works and the choices available to you.
What Information May Be Shared?
Depending on the program, the following information may be shared with healthcare providers involved in your care or between participating health plans:
- Healthcare services and visit history
- Treatments and procedures
- Medical conditions and diagnoses
- Prescription medication history
- Clinical information used to support your care
- Prior authorization information, such as approvals, denials, and effective dates
- Laboratory and diagnostic test results
- Claims information
Your health information is shared securely and protected by federal and state privacy laws, including HIPAA.
Sharing Information with Your Healthcare Providers
Beginning January 1, 2027, certain health information may be shared with in-network healthcare providers involved in your care. Information is only shared with those providers we have determined to have a record of caring for you. Before your health information can be shared, the provider’s identity must be verified, and the provider must meet other requirements to access your information.

- Reduces duplicate tests and procedures
- Gives providers a more complete picture of your health
- Supports informed treatment decisions
Your Choice
Your health information may be shared with in-network healthcare providers involved in your care unless you choose to opt out.
If you do not want your information shared, simply opt out by following the instructions below under Managing Your Preferences.
If you opt out and later change your mind, you can change your choice at any time to allow your information to be shared again.
Sharing Information Between Health Plans
Certain health information may be shared between health plans when you change health plans or have coverage through more than one health plan. This may include sharing information from a previous health plan when you enroll with Alignment Health Plan or between health plans that currently provide your coverage.
Benefits
- Supports continuity of care when changing health plans
- Helps your new health plan receive information from your previous health plan
- Helps coordinate information when more than one health plan provides your coverage
- Helps maintain more complete health records
- Reduces interruptions in care
Your Choice
Your health information will not be shared between health plans unless you give permission by choosing to opt in.
If you would like your information shared, simply opt in by following the instructions below under Managing Your Preferences.
